Person-Driven Plans

We provide customized services based on Person Driven Plan goals

Person-Driven Plans, also known as Person-Centered Plans, are increasingly regarded as central to developing a quality plan for the future success of individuals with disabilities. Vocational and Adult Services develops meaningful Person-Driven Plans for individuals elementary-age through adulthood. We use a positive, strengths-based approach in collaboration with individuals and their support team to develop a plan that includes key events, strengths, interests, activities, support needs, dreams, and plans. Information obtained at a Person-Driven Plan meeting is used to develop an effective and meaningful plan of action that is welcomed by the individual.
